Medicare sets a scheduled fee for the medical service provided. The doctor gets all of this fee. Most doctors charge more than the medicare scheduled fee set by the federal govt and this is the amount we have to pay from our own money. Sometimes doctors bulk bill for people on low income which means they just accept the Medicare scheduled fee and charge no extra fee on top.

The PRACTICE gets the entire Medicare Fee and the doctors in a practice negotiate what their % is. A new doctor will get around 45-55% of the fee, the rest going to the practice. There are very few sole doctor practices nowadays - they are all partnerships.
